Gift Card Drawing

We offer every respondent the opportunity to enter a drawing for a $25 Amazon gift card. We will have a drawing for every additional 25 responses and will notify winners via email.

Can you enter the drawing and still maintain response anonymity? YES! Entering the drawing is optional. If you do enter, your contact information is collected through a separate form, not linked to your survey responses.

When choosing winners, we attach a number to each response and then use this platform to randomly generate a number. The winner is then contacted by our team through email.
